Start with coverage: eligible Medicare enrollees pay $50 a month, and covered commercial plans can drop to $25 with a savings card. The cheapest legitimate ways to get a GLP-1

  1. Insurance or Medicare first. Since July 1, 2026, eligible people with Medicare drug coverage pay $50 a month for Wegovy (pen or pill), Zepbound KwikPen (not vials or single-dose pens) and Foundayo through the Medicare GLP-1 Bridge (until December 31, 2027). With employer or marketplace coverage that includes the drug, manufacturer savings cards bring Wegovy, Zepbound or Foundayo to as little as $25 a month. Check your formulary before paying cash.
  2. No coverage? Buy brand-name direct. Through TrumpRx.gov, LillyDirect and NovoCare Pharmacy, the FDA-approved drugs now cost: Wegovy pill $149 (1.5 mg; $199 at 4 mg, $299 at 9–25 mg), Foundayo $149 to start ($299 at maintenance), Wegovy or Ozempic pen $199 for the first two fills (through Dec 31, 2026) then $349, and Zepbound $299 (2.5 mg), $399 (5 mg), $449 (7.5–15 mg with refills within 45 days; otherwise $499–$699).
  3. Use telehealth for the prescription, not the drug markup. You still need a prescriber. Your own doctor is usually cheapest; online, Ro charges $39 for the first month then $74–$149 a month on top of those same cash prices. Compounded semaglutide or tirzepatide plans are listed further down, after the brand-name options, because they are not FDA-approved.

With Medicare or insurance

  • Medicare GLP-1 Bridge ($50/month): Medicare Part D (or Medicare Advantage with drug coverage), age 18+, and a BMI of 35+; or 30+ with heart failure with preserved ejection fraction, uncontrolled high blood pressure or stage 3a+ kidney disease; or 27+ with prediabetes, a past heart attack or stroke, or symptomatic peripheral artery disease. Not for people who already get a GLP-1 through their Part D plan, or who have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease (their Part D plan may cover a GLP-1 instead). Compounded GLP-1 plans (not FDA-approved)

Compounded semaglutide and tirzepatide are made by pharmacies for individual prescriptions and are not FDA-approved. FDA says they should only be used when an FDA-approved drug can't meet your medical needs. Price-wise: month-to-month compounded plans cost $165 to $399 a month, no cheaper than the brand-name prices above. Prepaid 3- to 12-month plans can cost less per month, but you pay upfront and most brands don't refund dispensed medication.

  • Lowest published 12-month tirzepatide total: HealthRX, $2,148 billed upfront ($179/mo). Gala also works out to $2,148 a year at $179/mo, but publishes no 12-month total.
  • Lowest headline price: CoreAge Rx, tirzepatide "starting at" $149/mo ($1,788 a year). Plan terms appear only at checkout, and its checkout page shows a different, lower price.
  • Most included in the price: MEDGm's 12-month tirzepatide supply, $3,228 ($269/mo), includes lab work, side-effect medication and shipping; it also sells compounded tablets and a brand-name route.

MEDGm bills $3,228 upfront ($269/mo) and is the only one that says lab work and side-effect medication are included; Klarna can split the payment. Found charges $239 per 28-day cycle on its 12-month cash term (about $3,107 for 13 cycles), or $179 on its insurance track plus copays.
